Data quality is everything. This form is designed to produce structured data that frontier labs use to improve their models. The more precise your inputs — exact prompts, raw outputs, clear reasoning — the more valuable your submission and the faster it gets accepted.

Do NOT include Protected Health Information (PHI). All prompts and clinical scenarios must be fictional, de-identified, or based on publicly available clinical vignettes. Do not include real patient names, dates of birth, medical record numbers, or any other HIPAA-protected identifiers.
